If you would like to speak with a Nurse Care Manager, contact your Coastal Medical practice to schedule a consultation.
Our clinical pharmacists will meet with you at your primary care office or over the phone, for a free medication consultation. They see patients one-on-one for medication reviews, medication teaching, medication management and collaborative chronic illness management as needed.
Our clinical pharmacists can:

- Look for effective ways to lower the costs of your prescriptions.
- Provide a detailed medication schedule.
If you are a Medicare patient, your Coastal primary care provider will ask that you schedule a Medicare Annual Wellness Visit in addition to your annual physical. The Annual Wellness Visit is a personalized, conversational visit that gives you time to discuss your health in detail with a Coastal Medical clinician. It is not an exam. Most patients, regardless of their health status, find value in their visit.
Patients with heart, lung or sleep conditions can visit the Coastal Specialty Care Center at 900 Warren Avenue, Third Floor. Pulmonologists, cardiologists and clinical providers will help you manage your specific condition.
