CREDIT CARD ON FILE POLICY
Please check-in with the receptionist upon arrival. Please have your insurance card available for review and please check that we have your current address and telephone number. A co-payment may be required. Co-payments are due upon check in. We accept cash, check, Visa and MasterCard for payment. We require a credit card to be kept on file for all patients (Debit, FSA or HSA cards are also accepted).
When your visit is complete, please “check out” with the receptionist. Appointments or tests may need to be scheduled and final instructions may need to be given. A co-payment may be required. Co-payments are due at the time of the service. We accept cash, check, Visa and masterCard for payment.
If we are your primary care physicians and you have an insurance plan that requires “referrals,” you may send a request through your Patient Portal or call this office. Your physician will review your request, and if approved, we will forward the referral to the specialist or you may come to the office to pick it up. If your primary care physician is not part of our group and you are seeing one of the physicians for specialty services, you must get a referral before your visit.
In order to provide the best possible care to our patients in the hospital and in the office, we have dedicated hospital physicians who oversee the daily hospital care of our patients. These physicians have access to your history and are in communication with your primary care physician. Upon discharge you will be scheduled to see your physician in the office for follow up care.
