In addition to our clinicians, is Waterman Pediatrics’ Care Team. We provide coordinated, individualized patient centered medical care to all of our patients. We are proud to have earned the distinction as a NCQA Recognized Level 3 Patient Centered Medical Home.
Periodically, we send email notifications to our patients and families in the event of office closings due to inclement weather, flu clinic signup and other office related information. This list will never be used for any purpose other than office related issues and will never be shared outside of Waterman Pediatrics. CHADIS Patient Screenings
You will now be able to complete your questionnaires online prior to your appointment through the CHADIS (Child Health and Development Interactive System) website. We are hopeful that this will make your visit more efficient. CHADIS will keep all your information completely confidential. We would love for all families to register their children. Once you’re registered, the appropriate questionnaire for your child’s age will be available under “take questionnaire.” Only certain ages prompt questionnaires so don’t be concerned if you don’t see one.
